About the Role:

As an LPN Licensed Practical Nurse, you will provide high-quality, patient-centered care to our residents, working closely with our interdisciplinary team to develop and implement individualized care plans. With your clinical expertise and exceptional communication skills, you will build trusting relationships with our patients and their families, ensuring their physical, emotional, and social needs are met.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians

• Perform vital signs, monitor patient progress, and report changes to the healthcare team

• Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming

• Develop and maintain accurate and detailed patient records

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Promote a positive, supportive environment that fosters respect, dignity, and safety for our patients and colleagues
