Remote RN Care Management Lead - Stars Initiative

Humana Inc. is seeking a Manager, Care Management to lead a remote team of Stars-focused care coaches. You will guide associates, promote high-quality member-centered care, and adapt strategies as Stars priorities evolve.
This role reports to the Associate Director of Care Management and drives performance in outreach, care-gap closure, and member outcomes. The ideal candidate has an active RN license, 5+ years RN experience, 3+ years in case management, and 1+ year of data-driven leadership.
