In accordance with The Massachusetts Pay Transparency Act, BNHC provides reasonable pay range for each posted position. Actual compensation will be based on multiple factors such as relevant experience, education and training to determine offered rates. This range represents the organization's good faith estimate of the possible compensation at the time of posting.

POSITION SUMMARY

The Behavioral Health Integration (BHI) Clinician is a key member of a collaborative care team within a multicultural Community Health Center. This role works closely with patients, their families (when appropriate), primary care providers, nurses, community health workers (CHW), and other support staff to deliver integrated behavioral health services.

The behavioral health clinician (BHC) provides urgent consultation and bio-psychosocial assessments, brief interventions, counseling, education, and linkage to services. The goal is to improve access to care, support self-management, and enhance both physical and psychological well-being. Patients are typically referred via warm hand-offs from medical providers and may be high-risk due to complex medical conditions, frequent emergency room use, or behavioral, social, or environmental challenges.

PRINCIPAL DUTIES AND RESPONSIBILITIES

  • Conduct culturally competent assessments, consultations, and brief interventions for psychological and psychiatric concerns using DSM-5 criteria.
  • Evaluate crisis situations and implement appropriate interventions, including suicide and homicide risk assessments.
  • Collaborate with the primary care team to develop care management strategies such as behavior modification, disease management, and patient education.
  • Provide clinical services for a wide range of behavioral health and behavioral medicine needs, including:
    • Stress and mood management
    • Anxiety and panic interventions
    • Substance use assessment and brief intervention
    • Psychotropic medication response monitoring
    • Health behavior change and chronic disease management
    • Medication adherence support
    • Weight and chronic pain management
    • Smoking cessation
    • Sleep hygiene and insomnia treatment
    • Management of high medical utilization
    • Psychosocial aspects of chronic illness
    • Utilize evidence-based therapeutic techniques, including:
      • Psychoeducation
      • Cognitive Behavioral Therapy (CBT)
      • Motivational Interviewing
      • Mindfulness-based interventions
      • Relaxation training
      • Solution-focused strategies
      • Goal setting and problem-solving
      • Communication and wellness promotion skills
    • The BHI Clinician will be assigned a small patient panel and is expected to deliver eight (8) hours of outpatient treatment weekly.
    • Facilitate collaboration between BNHC psychiatric providers and primary care providers for co-management of complex cases.
    • Coordinate with community-based mental health, substance use, and social service providers to ensure comprehensive care.
    • Participate in clinical treatment team meetings to support integrated care planning.
    • Attend administrative meetings as assigned.
    • Assist with data collection and outcome measurement as needed.
    • Document all services provided in the electronic health record (EHR) in a timely (within 72 hours of encounter) and accurate manner.
    • Provide supervision to assigned behavioral health staff as assigned
    • Perform other duties as assigned.

    ADDITIONAL REQUIREMENTS

    • Strong knowledge of behavioral medicine and evidence-based treatments for mental and physical health conditions.
    • Ability to conduct brief, focused clinical assessments and interventions in a fast-paced primary care setting.
    • Comfortable working in interdisciplinary, team-based care environments.
    • Excellent communication and collaboration skills.
    • Solid understanding of psychopathology and psychopharmacology.
    • Ability to design and implement clinical pathways and protocols for chronic condition management.

    Qualifications

    QUALIFICATIONS:

    Candidates must be licensed or currently seeking licensure in one of the following: Social Work (MSW, LCSW, LICSW); Mental Health Counseling (LMHC) or Psychology (PhD, PsyD)

    Language and cultural diversity are preferred, including Cape Verdean Creole, Portuguese, Haitian Creole, French, or Spanish.

    WORKING CONDITIONS:

    A busy community health center located in a downtown business district. Occasional long hours may be required.

    PHYSICAL REQUIREMENTS:

    Physical demands requiring lifting of small pieces of equipment and/or boxes not to exceed 20 pounds. Mostly sedentary, with interoffice walking required. Visual acuity sufficient for frequent reading. Hearing acuity sufficient for holding conversations with or without audio devices.
