Nurse Care Manager - RN or LPN

Position: Nurse Care Manager - RN or LPN Location: Detroit Job Id: 325-2022 # of Openings: 1 Summary The Nurse Care Manager is responsible for overseeing the work of the Medical Assistants, Phlebotomists and working with existing Advantage Health providers to coordinate patient care in order to achieve improved patient health outcomes. The Nurse Care Manager reports to the Medical Director for guidance, supervision, and evaluation. Functions primarily as a member of the general interdisciplinary (IDT) service teams of each clinic. Duties and Responsibilities Responsible for supervisory functions including hiring, evaluations, discipline, skills training, assigning, and directing work, addressing complaints and resolving problems for Medical Assistant and Phlebotomist staff Oversee the following: Staff and Patient Vaccine initiatives Care/Procedure Coordination efforts Provide treatment recommendations and strategies to achieve care plan goals through regular team meetings and case conferences with providers Establish and maintain a relationship with AH clinicians and site leadership through routine communication Implement clinical policies and procedures in conjunction with Medical Director and other members of Senior Management Team Requisition clinic personnel, supplies, and equipment to assure efficient site operation Recommend purchase of new equipment, inspection of clinical sites, and consideration of AH policies and knowledge of community needs and resources Review and analyze clinical activities and data to aid in operational and service delivery to improve utilization Work with Community Health Workers and Clinical Leadership to develop and review patient education tools Other duties as assigned Qualifications Language Ability Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public. Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Computer Skills Knowledge of Microsoft Applications – including Microsoft Excel and Microsoft Outlook, internet search engines, and the EHR. Education/Experience Associates Degree or higher, Active Michigan Nursing Licensure and 1-3 years related experience in nursing, care‑coordination, and management of people. Individuals with experience in Health Homes or Patient Centered Medical Home (PCMH) models of care will be highly favored. Specialized Training Working knowledge of care management/population management systems Working knowledge of care management models and ability to apply to patients in a community health center setting Previous experience working with an electronic medical record system Ability to work with individuals from diverse cultures, varied professional backgrounds and patients with multiple chronic conditions Previous experience working in a PCMH clinic or Health Home – a plus Certificates and Licenses State of Michigan license as a Registered or Practical Nurse Valid driver’s license Active life support certification /BLS HIPAA training Physical Demands While performing the duties of this job, the employee is regularly required to sit and talk/hear. The employee is frequently required to use hands and reach with hands and arms. The employee is occasionally required to stand, walk, stoop, kneel, crouch or crawl, and taste/smell. The employee must occasionally lift and/or move up to two pounds. Specific vision abilities required by this job include close vision, distance vision, and ability to adjust focus. Work Environment Frequently exposed to moderately adverse working conditions in performing certain patient care activities.
