Must have graduated from an accredited school. Minimum one year of experience. Current state license as a Registered Physical Therapist / PT or be actively pursuing licensure. Current CPR certification. Other requirements as determined by client facility. This includes direct patient care, evaluating medical condition, functional capabilities, limitations, and potential for rehabilitation; establishing and administering a treatment program with specific goals; setting realistic and achievable goals; documenting and recording patient condition; and educating patients and families. Develop a plan of care for each patient. Provide skilled physical therapy services/interventions in accordance with physician orders. Assess patient needs, plan for, evaluate, and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient.
