Job DescriptionEDUCATION:
High school graduate or GED preferred. All applicants must be at least 16 years old. Completion of a 75-hour nurse aide course (or completed TNA course and challenging/passing the State C.N.A. test during pandemic as declared by the CDC) is required. Expired C.N.A certificate holders may challenge the test within __ month of employment. Nursing assistants must be able to read, write and follow written and oral instructions in English. Failure to maintain certification as a nursing assistant will result in termination.
WORK EXPERIENCE:
Previous experience as a certified nursing assistant in a long-term care setting is desired.
CONTINUING EDUCATION REQUIREMENT:
All nursing assistants are required to complete 12 hours of continuing education on an annual basis. Failure to complete the 12-hour requirement each year or any other local, state or federal requirement may result in termination.
GENERAL JOB SUMMARY:
A nurse aide is responsible for providing personal care and assistance in the facility. Nurse aides may be called upon to help residents in and out of bed, and with dressing, grooming, personal hygiene, toileting, feeding and bathing. Nurse aides will be called upon to take resident vital signs as requested by the Charge Nurse. Nurse aides are responsible for observing and reporting changes in residents’ physical and emotional conditions to the supervisors. Excellent communication and team skills are needed. May be required to work a variety of shifts depending on resident needs.
DUTIES AND RESPONSIBILITIES:
- Assist clients with daily activities, including but not limited to: bathing, dressing, serving meals, helping clients eat, turning clients and ambulating clients
- Bathe or assist residents in bathing via a bed bath, tub, shower, or whirlpool.
- Examine clients for bruises, blood in urine or other injuries/wounds.
- Check and record patient vital signs.
- Collect information about conditions and treatment plans from caregivers, nurses, doctors and family members.
- Perform or assist residents with personal grooming needs such as shaving, hair care, oral hy
- giene, denture care, skin care, toileting, and cosmetics.
- Assist residents in dressing and undressing as needed in clothing appropriate to the time of day and season of the year.
- Assist resident in caring for and securing personal articles, labeling clothing, etc.
- Clean and sanitize client areas, including changing bed sheet.
- Assist residents with before and after meal routines such as washing face and hands, transporting to and from the dining area or preparing bedside environment for in room dining.
- Provide prompt meal-time assistance (checking diet card for accuracy, position food and utensils for resident convenience, help with cutting foods, buttering bread, pouring liquids, etc.)
- Provide partial or total feeding assistance and encouragement as needed.
- Report appetite changes and request substitutions when necessary.
- Record observations on meal intake records.
- Provide adequate hydration such as passing ice, fresh drinking water, and other fluids.
- Provide between meal nourishment when indicated (distribute supplemental nourishment as assigned, aid when needed, accurately record and report intake of nourishment.)
- Assist resident in all elimination needs (bedpan urinal, commode, or bathroom assistance.)
- Participate in the facility incontinence program.
- Perform perineal care after each toileting or episode of incontinence per facility protocol.
- Perform catheter care per facility protocol.
- Record bowel habits and urine outputs as directed.
- Collect specimens as directed.
- Reposition immobile residents at time and in a manner as instructed.
- Assist residents in transferring from bed, chair, wheelchair, toilet, etc. using appropriate equipment and following the principles of proper body mechanics according to the care plan instructions for each resident.
- Assist residents in exercise (active/passive range of motion, ambulation, etc.) as indicated by the supervisor and individualized care plan.
- Help restore independence through teaching and encouragement in ADL skills (eating, dressing, ambulation, toileting, etc.)
- Help prevent complications of decubitus ulcers and contractures by the replacement of positioning aids, protective padding, etc.
- Stay up to date on CNA training, policies, and procedures.
- Ability to reasonably conform to all rules and policies of this facility.
- Other duties as assigned.
PHYSICAL ACTIVITY REQUIREMENTS:
Lifting Requirements:
The Certified Nursing Assistant will frequently be called upon to lift various items between 10 and 50 pounds. Certified Nursing Assistants will frequently be required to lift over 50 pounds while transferring a non-ambulatory resident weighing between 100 and 160 pounds. Assistive devices are available for resident transfers.
Carrying Requirements:
The Certified Nursing Assistant will frequently be called upon to lift various items between 10 and 50 pounds. Sight Requirements:
The Certified Nursing Assistant is required to have vision corrected to average to assess and respond to resident needs. Specific vision abilities required by this job include close vision, depth perception and ability to adjust focus.
Hearing Requirements:
The Certified Nursing Assistant is required to have normal range of hearing, as corrected and in order to respond to resident/staff/physician/family communication.
Socialization Requirements:
The Certified Nursing Assistant is required to work with all other persons in the facility and the ability to get along and socialize with others including residents and families is essential. Good communication skills are required.
Other Physical Requirements:
While performing the duties of this job, the Certified Nurse Assistant is frequently required to use hands to finger, handle, or feel; reach with hands and arms; and talk or hear. Will be called upon to frequently perform twisting, bending, squatting, kneeling, or crouching when assisting with resident needs. The employee is frequently required to reach with hands and arms.
