Here, you will: Spearhead detailed patient assessments to devise individualized Plans of Care. Offer expert physical therapy in the Home Health sphere as per physician directives. Collaborate closely with the broader team, ensuring optimal patient outcomes. Streamline communication with referring physicians, maintaining thorough and timely documentation. Job Requirements Graduate of an accredited school of physical therapy. Active Physical Therapy License in the Commonwealth of Virginia. Prior home health care experience. Current CPR certification.
