The Private Duty RN will provide skilled nursing care in patients' homes, focusing on chronic illness management under the guidance of a physician and patient/caregiver input. This role involves comprehensive patient assessment, developing and implementing care plans, and maintaining accurate documentation using an Electronic Health Records (EHR) system. The nurse will collaborate with the home health team to ensure compassionate, culturally sensitive care that promotes patient comfort and safety.

Responsibilities

• Conduct initial and ongoing patient assessments using OASIS data elements
• Develop, implement, and adjust individualized care plans with patient and physician input
• Document all observations and treatments accurately in compliance with agency policies
• Maintain patient confidentiality in accordance with HIPAA regulations
• Provide emotional and physical comfort considering patient rights and cultural background
• Communicate changes in patient condition and unusual reactions to physicians promptly
• Participate in team meetings, case conferences, and performance improvement activities
• Adhere to all agency policies and procedures at all times
• Visit patient for 4 hours per week

Required Qualifications

• Minimum 1 year of nursing experience
• Valid Nursing License in the state of Arizona
• Level 1 AZ Fingerprint Card

Schedule

• Part Time Days - 24 hours per week

What is the EEOICPA?

The EEOICPA (Energy Employees Occupational Illness Compensation Program Act) under the United States Department of Labor has a free (to eligible energy workers) program to completely cover the medical costs of energy workers such as uranium miners, millers, and haulers or beryllium workers from the Cold War era.

This program also provides monetary compensation for damages suffered. Illnesses include, but are not limited to radiation-induced cancers and beryllium-related illness.
