Location: Inpatient Unit at OSU Medical Center Every third weekend required Holiday rotation required About the Role Join our Hospice House/Inpatient Unit team as a Registered Nurse and provide compassionate, patient-centered care to individuals with life-limiting illnesses. you'll support patients and their families throughout their hospice journey while working closely with an interdisciplinary team in a collaborative environment focused on comfort, dignity, and clinical excellence. Key Responsibilities Provide skilled nursing care using the nursing process, including assessment, planning, implementation, and evaluation. Assess and address patients’ physical, emotional, psychosocial, and spiritual needs. Develop, implement, and regularly update individualized plans of care in collaboration with patients, families, and the interdisciplinary care team. Monitor changes in patient condition and communicate significant updates to physicians to ensure appropriate levels of care. Evaluate and adjust nursing interventions, medications, and care needs as patient conditions change. Participate in interdisciplinary team meetings, care conferences, and patient admissions. Maintain accurate and timely documentation of assessments, interventions, physician orders, and patient status in the electronic medical record. Provide thorough patient handoffs using effective communication methods, including SBAR, to promote continuity of care. Provide guidance and oversight to personal care specialists, volunteers, and other caregivers to ensure care is consistent with the established plan. Collaborate with other departments and team members to support exceptional patient care. Participate in quality improvement initiatives designed to enhance patient outcomes and organizational excellence. Serve as a preceptor for new team members when assigned. Qualifications Current Registered Nurse (RN) license in good standing with no disciplinary actions. Current CPR certification. Certified Hospice and Palliative Nurse (CHPN) certification preferred; support is available for eligible employees pursuing certification. Strong clinical assessment, symptom management, and care planning skills. Experience documenting patient care in an electronic medical record system. Valid Ohio driver’s license, reliable transportation, and proof of auto insurance. Ability to successfully complete a criminal background check and 10-panel drug screening.
