Summary This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval – award amount (up to $200‑000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants are ineligible to apply. Qualifications Applicants pending completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements United States Citizenship: Non‑citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d)– and7407(d). Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree programs; today the sole degree is Doctor of Pharmacy. Graduates of foreign pharmacy degree programs meet the educational requirement if they provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet‑Based Test (TOEFL iBT). Licensure: Full current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico) or the District of Columbia. The pharmacist must maintain current registration if this is required for maintaining full‑current‑and‑unrestricted licensure. A pharmacist who has ever had any license(s) revoked, suspended, denied, restricted, limited, or placed on probationary status may be appointed only in accordance with VA Handbook 5005– Part II– Chapter 3– section B– paragraph 16. Exception: Non‑licensed pharmacists who otherwise meet the eligibility requirements may be given a temporary appointment at the entry level as a Graduate Pharmacist under the authority of 38 U.S.C. § 7405(c)(2)(B). May qualify based on being covered by the Grandfathering Provision described in the VA Qualification Standard for this occupation (applicable only to current VHA employees in this occupation who meet the criteria). Grade Determinations: In addition to the basic requirements, the following criteria must be met when determining the grade of candidates. (KSAsmust be clearly reflected in resume under description of duties in order to be found qualified for this position/grade level). GS‑11: Pharmacist – Experience, Education, and Licensure. GS‑12: Clinical Pharmacist – Experience or Education. Candidates must meet one of the following: One year of experience equivalent to the next lower grade level– ORCompletion of an ACPE‑accredited Pharm.D. program. Demonstrate the following Knowledge, Skills and Abilities (KSAs): Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to patients and health‑care staff. Knowledge of laws, regulations, and accreditation standards related to scheduled and non‑scheduled drug distribution and pharmacy security. Skill in monitoring and assessing drug‑therapy outcomes, including physical assessment and interpretation of laboratory and diagnostic parameters. Preferred Experience: At least one year of recent outpatient or ambulatory care clinical pharmacy practice in a health‑system or VA setting, providing direct patient care under collaborative practice agreements or scope of practice. Prior experience working within a VA or integrated health‑system environment, including familiarity with formulary management, prior authorization processes, population health tools, and interdisciplinary team‑based care. Strong experience with patient counseling and telephone or virtual care follow‑up, medication adherence interventions, and comprehensive medication management for a diverse adult population. PGY1 pharmacy residency completion, or ≥ 3years ambulatory care and primary care disease‑state management practice (e.g., diabetes, hypertension, dyslipidemia, anticoagulation), or BPS board certification (BCPS etc.) as accepted alternatives. Physical Requirements Typical occupational physical demands include standing, lifting, carrying, sitting, stooping, bending, pulling, and pushing. Employees may be required to wear personal protective equipment and participate in annual TB screening or testing as conditions of employment. Work Environment Work is performed in an office/clinic setting requiring normal safety precautions, adequate lighting, heating, and ventilation. The environment requires someone capable of handling multiple tasks concurrently in sometimes stressful situations. Duties Allergy assessment – ADR information – lab value assessment – elimination of duplicate/obsolete orders. Review drug regimens for therapeutic effectiveness and appropriateness per supervisor review and professional practice evaluation. Monitor and counsel patients for unintended drug‑related problems (e.g., adverse drug reaction, medication error, noncompliance). Communicate with other members of the health‑care team to develop drug therapy assessments. Provide proactive medication interventions. Provide accurate professional, technical, and procedural information to the team. Active participation in emergency code and pre‑emergency situations. Attend TMS trainings, departmental competencies, and required readings. Sterile compounding with strict measures for documenting and delivering controlled substance sterile products per hospital and departmental policies. Proficiency in operating relevant VistA computer packages (e.g., CPRS, CPOE, Controlled Substance, Outpatient, Inpatient, ProgressNote, Consult, Allergytracking, HospitalInformation, Laboratory, TimeandAttendance).
