Pay Range: $65,000 - $73,000, Based on experience
Schedule: Monday - Friday, 8:00am - 5:00pm , Occasional evenings and weekends

What You Bring (Requirements)

• Education: Masters’ Degree from an accredited school of social work required.
• Must have medical clearance for communicable diseases and up-to-date immunizations after having direct participant contact.
• Must clear background check.
• Must have a valid driver’s license and have means of transportation.
• Be legally authorized, currently licensed, registered or certified in Kentucky .

What You’ll Be Responsible For

• Acts as and engaged member of the Interdisciplinary Team (IDT). Regularly attends meetings and provides meaningful input.
• Performs in-person initial assessments for enrolling participants to obtain a complete psychosocial history, which may include descriptions of cognitive status, social supports, family dynamics mental health and substance dependency and other issues and needs. Coordinates with the IDT to develop a comprehensive plan of care for each participant.
• Support residents’ physical and emotional well-being by building meaningful connections and delivering compassionate, person-centered care.
• Monitor and report changes in residents’ condition to the nursing team to ensure timely and appropriate care
• Maintain a clean, safe, and comfortable environment while following all care plans, safety protocols, and infection control standards

Senior Community Care of Kentucky PACE (Northern) provides healthcare and supportive services for individuals age 55 and older, helping them live safely, comfortably, and independently in their own homes and communities. As a Program of All-Inclusive Care for the Elderly (PACE) provider, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs. Located in the welcoming community of Florence , PACE is part of the greater Northern Kentucky region. Just minutes from downtown Cincinnati, our location offers access to a vibrant riverfront city with dining, arts, and cultural attractions. We also provide convenient access to the scenic Ohio River corridor, with parks, walking trails, and riverfront views.
