Physical Therapist

Role: Licensed Physical Therapist providing in-home and senior living care to older adults, utilizing a flexible, patient-centered approach. Responsibilities: Conduct evaluations, develop personalized treatment plans, deliver evidence-based therapy to support mobility, independence, and wellness. Document care accurately in EMR, collaborate with interdisciplinary teams, educate patients and caregivers, and incorporate telehealth when appropriate. Qualifications & Skills: Valid PT license, degree from an accredited program, proficiency with EMR and computers, current CPR certification, and ability to meet physical demands including lifting, repositioning patients, and driving.
