Home Health Case Manager

Key Responsibilities

- Provide skilled nursing care and comprehensive patient assessments in the home setting
- Develop, implement, and manage individualized care plans
- Perform wound care, wound vac management, and central line care
- Administer IV therapy and perform venipuncture procedures
- Manage Pleurex catheter care and other specialized treatments
- Educate patients and families on disease management, medications, and care plans
- Coordinate care with physicians, therapists, caregivers, and interdisciplinary teams
- Accurately document patient visits and updates within the EMR system
- Monitor patient progress and respond effectively to changes in condition
- Manage daily schedules efficiently while maintaining quality patient care
- Minimum 1 year of Home Health nursing experience required
- Prior visiting nurse experience strongly preferred
- First-time travelers will not be considered
- Wound vac management and wound care
- Active Illinois RN License or Compact RN License
- American Heart Association BLS Certification
- Valid driver's license
- Reliable vehicle with active auto insurance
- Technology Experience
- EMR experience preferred, especially with Epic
- Experience with Sapphire IV pumps preferred
- Mileage: Reimbursed
- Consistent weekday schedule with work-life balance
- No on-call or holiday requirements
- Manageable daily patient caseloads
- Strong leadership and educational support
