Registered Nurse- Hospice

Provide Hospice specific nursing care to patients in home, nursing homes and assisted living facilities. Provide initial and on-going counseling/education regarding illness, disease process, and/or bereavement support. Perform comprehensive Nursing Assessment on newly admitted patients. Evaluate data collection, formulate plan of care and establish specific goals for the patient and family/support system. Work closely with physician communicating pertinent observations. Regularly reassess the patient and update the care plan accordingly. Coordinate supplies and durable medical equipment, assuring delivery in a timely and cost efficient manner. Coordinate inpatient, respite, or acute admissions as well as outpatient services and treatments. Serves as patient/family advocate Maintain patient/family dignity.

Current GA RN license. At least two years of experience, preferably in hospice setting. Extensive travel in service area (must have clean driving record).

Location: Archbold Medical Center · HOSPICE Schedule: Full Time, Day Shift, 8:30am-5:00pm Monday-Friday with weekend on-call rotation.
