Palms Home Care - Reports To: Clinical Manager Our home health care agency is expanding and searching for dedicated Home Health RNs for FT and PRN positions. Responsibilities • Perform comprehensive patient assessments and develop individualized care plans in collaboration with the care team. • Complete evaluation tasks, including reviewing medication and vital signs. • Coordinate care by making appropriate referrals or documenting clinical rationale when referrals are not indicated. • Communicate effectively with the clinical manager and interdisciplinary team regarding patient status and care updates. • Educate patients and caregivers to promote independence and improve health outcomes, including training in self-care techniques. • Coordinate with Physicians, Physical Therapists, Occupational Therapists and other individuals in the patient’s care plan. • Supervise and support LPNs to ensure high standards of care. • Perform additional duties as needed to support patients and team operations. Qualifications • Current Registered Nurse (RN) license in the state of Florida • Must have at least 1 year experience as a home health nurse • Excellent knowledge of nursing care methods and procedures • In-depth knowledge of health and safety guidelines and procedures • A team player with excellent communication and interpersonal skills • Outstanding organizational and multi-tasking skills • Patient with excellent problem-solving skills • Familiar with EHR such as Kinnser. • Ability to work independently, manage a daily schedule, and travel between patient homes Benefits for Fulltime Staff: • Dental insurance • Health insurance • Vision insurance • Paid Time Off • Paid Holidays • Auto allowance This role requires a Level 2 background screening in compliance with Florida AHCA regulations prior to employment. #J-18808-Ljbffr
