Physician Assistant / Family Nurse Practitioner

We are hiring a Physician Assistant (PA-C) or Family Nurse Practitioner (FNP-C) to join our team.

What You'll Do:

• Provide thorough, personalized primary care to FPC members
• Build meaningful, long-term relationships with a limited patient panel
• Communicate directly with patients by phone, text, and email
• Diagnose and manage chronic conditions with an emphasis on prevention and long-term health
• Support patients with nutrition, exercise, sleep, stress management, and other lifestyle goals
• Perform procedures and expand your clinical skills within your scope of practice
• Collaborate with a supportive clinical team to continuously improve patient care

Requirements

• Active PA-C or FNP-C license
• Fluent Spanish, spoken and written, required
• Strong communication and interpersonal skills
• Compassionate, professional approach to patient care
• Ability to handle sensitive information with confidentiality
• Strong follow-through and care-planning skills
• Passion for preventative and lifestyle-focused medicine
