Trustbridge Hospice, part of Empath Health, seeks an organized and friendly Care Navigation Manager for our West Palm Beach team. This on-site full-time role (Mon-Fri, 11:00 a.m.–7:30 p.m.) supports Admissions and guides patients and families through the hospice referral and access process with empathy and efficiency. The manager leads intake and care navigation staff, ensures quality documentation in the EHR, and coordinates initial visits and admissions with compliance to HIPAA and regulatory #J-18808-Ljbffr Trustbridge
