4004 Aetna Medicaid Administrators is seeking a Case Management Coordinator to support members through care management tools and education within the ABH-Louisiana Care Management Program.
You will evaluate referred members, coordinate care plan activities, and collaborate with case managers and medical directors to overcome barriers and improve health outcomes. This full-time role requires 50% travel and offers comprehensive benefits including medical, dental, and vision coverage.
