Seven Healthcare are seeking a skilled Physical Therapist for a travel assignment in New York. This role involves providing case management and direct patient care within a supportive team environment, delivering comprehensive physical therapy in the home setting. Candidates must have experience with Oasis and be comfortable working in a dynamic environment supporting the Finger Lakes Team.

Key Responsibilities of the Physical Therapist
- Develops and revises plans of treatments and discharge plans in coordination with patients, families, physicians, and care teams
- Initiates appropriate rehabilitative programs in the home setting
- Provides supervision of PTAs to ensure patient safety and compliance with care plans
- Consults and educates patients, families, and other team members on disease process, self-care and preventive strategies
- Monitors patient progress and adjusts physical therapy programs as needed
- Ensures completion of clinical records for compliance and service delivery
- Communicates effectively with patients, families, and care staff to promote positive interactions
- Supports care management philosophy for quality and cost-effective patient outcomes
Requirements of the Physical Therapist
- B.S., M.S. or D.P.T. degree in Physical Therapy
- Possession of a current New York State License in Physical Therapy
- BLS certification (AHA only)
- Minimum of one year experience in Long Term Care/Home Care setting as a Physical Therapist
- Recent Home Care experience within the last year
- Good verbal, written communication, and interpersonal skills
- Proficiency in physical therapy procedures
- Experience with Oasis is mandatory
