Minimum Requirements

• Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Washington State Physical Therapist license is required to start the assignment.
• BLS/CPR certification required.
• Home health experience required. Experience with SOC, evaluations, ROC, and recertifications preferred.
• Experience with HCHB electronic medical record preferred.
• Ability to cover patients within a 50 mile radius of the primary office location.
• Ability to complete weekly 40 hours as scheduled.
• Business casual dress required.

• Facility type: Home Health.
• Shift name: Days.
• Shift details: Days 0800-1600 (schedule noted as Days 8:00 am - 4:30 pm in client notes).
• Weekly guaranteed hours: 40.
• On call/weekend requirement: No.
• Coverage area: 50 mile radius; mileage reimbursed between visits from first to last patient.
• Primary responsibilities: Start of Care (SOC), evaluations, return of care (ROC), recertifications, and subsequent visits.
