Home Health AideThe Home Health Aide role is primarily responsible for assisting in the implementation of the Lifespark Experience and the Life Care Management model including but not limited to assisting with Life Plan, Care Assignment activities, and delegated duties as well as honoring clients' rights, customer service and communication, emergency preparedness, and safety.Schedule: Shifts are 4-hours minimum, live-in shifts available. 1 and 2-hour visits available Wage: $18.00 - $24.00 – plus mileage reimbursement 72.5 cents per mile Multiple openings: Minnetonka, Wayzata, Chanhassen, Eagan, Apple Valley, Burnsville, St. Paul, Woodbury, White Bear Lake, Lino LakesPosition Functions and ResponsibilitiesLifespark ExperienceAssist with implementation of Lifespark Experience including items identified on the Life PlanReview Life/Care Plan and understand expectationsProvide companionship and socializationAssist with activity and hobby fulfillmentProvide transportation (as needed)Other Duties as AssignedAssist clients with activities of daily living and delegated tasks according to clients' care plan, needs, and wishes.Review Care Plan and follows what is expectedPersonal Cares (bathing, dressing, grooming, toileting, skin care, etc.)Lifting, Transferring (pivot transfer, 2-person, Hoyer lift)Range of Motion & PositioningExercise & AmbulationMeal Prep and assist with feeding (as needed)Homemaking (light housekeeping, laundry, personal shopping)Vital SignsMedication Reminders and AdministrationSpecial Tasks as directed by LCMObserves and Reports Client Status updates and changesFollows all documentation requirementsOther Duties as AssignedMake the clients feel comfortable and well-cared for.
