Responsibilities
- Assess and treat patients requiring physical therapy services within a skilled nursing setting
- Develop, implement, and monitor individualized therapy plans, including gait training, strengthening, and functional mobility
- Perform initial evaluations, document progress, and adjust treatment plans based on patient response and physician orders
- Collaborate with an interdisciplinary team to optimize patient outcomes and discharge planning
- Maintain accurate, timely documentation and ensure compliance with facility policies and regulatory requirements
- Promote patient safety, infection prevention, and appropriate equipment use during therapy sessions

Requirements
- Active Physical Therapist license in Pennsylvania
- Doctor of Physical Therapy (DPT) degree or equivalent
- Ability to work in a skilled nursing / long-term care environment
- Strong communication and teamwork skills with the interdisciplinary care team
- Availability to work the guaranteed 35-hour weekly schedule on the 8:00 AM-4:30 PM shift
