Hospice Registered Nurse Case Manager

Role We are seeking an experienced Hospice RN Case Manager to provide care to patients in a mix of long‑term care facilities and homes. Location Coverage will be in Chelsea, Winthrop, Everett, MA and surrounding areas. Qualifications Requires a minimum of 1 year of Hospice RN experience with a Medicare‑certified agency. Responsibilities Provide quality nursing care to identified Hospice patients. Perform direct patient care, coordinate services, and advocate for patients. Engage with patients, families or significant others, and the community. Comfortable handling end‑of‑life issues, death, and dying. Schedule Full‑time, Monday‑Friday.
