Physical Therapist - Travel Contract

Provide assessments, develop treatment plans, deliver skilled interventions, and document patient progress to improve mobility, endurance, pain management, and functional independence. Minimum Qualifications

• Master‘s or Doctoral degree in Physical Therapy from an accredited program.
• Active Kentucky Physical Therapy license required to start the assignment.
• Previous home health experience required, including use of HCHB electronic medical record.
• BLS certification from AHA or ARC may be required.
• Experience providing patient assessments, developing home-based treatment plans, and documenting progress in home health settings.

Preferred Qualifications

• Prior experience working for LHC or similar home health organizations. (Preferred)
• Experience managing a caseload across regional coverage areas such as Elizabethtown-Hardin County and Hardinsburg-Meade County. (Preferred)

Assignment Details

• Shift hours: 08:00 to 16:00 as documented by client schedule.
• Workdays: Monday through Friday.
• Weekly guaranteed hours: 32.
• Productivity expectation: 30 points.
• Coverage area includes Elizabethtown-Hardin County and Hardinsburg-Meade County.
• Mileage reimbursement provided between patient visits only, from first patient to last patient of the day.
• On-call and weekend requirement: no.

Impacting Patient Care

Provide skilled physical therapy services in patients’ homes to support recovery from injury, promote mobility, and manage chronic conditions. Collaborate with interdisciplinary home health teams to coordinate care and improve outcomes. Maintain accurate and timely documentation in the HCHB electronic medical record to support continuity of care and regulatory compliance.
