Unit-Based Pharmacist - Patient Care & Rounds

Hackensack Meridian Health is seeking a Unit Based Pharmacist to join our patient-centered team. This role integrates pharmacists into critical care with responsibility for clinical and distributive duties within the pharmacy department. The candidate will validate medication reconciliation on admission and before discharge, educate patients, and work with physicians to optimize therapy while managing costs.
