Compassionate Hospice Aide | Certified Home Health Caregiver
Northwell Career Site
Northwell Career Site is seeking a Home Health Aide to support primary care patients in a hospice care setting across Queens and Nassau counties. You will follow the plan of care, assist with ADLs, document care, and participate in team rounds to ensure patient comfort and safety.
The ideal candidate has a high school diploma, NYSDOH-certified HHA training, and at least one year of hospice or home health experience. A valid driver's license is required to travel between patient locations.
