Monitoring and Adjustment: Regularly assessing residents' progress and making necessary adjustments to treatment plans. Education: Educating residents and their families on strategies for fall prevention, pain management, and home exercise programs. Collaboration: Working closely with nurses, doctors, and other healthcare professionals to ensure comprehensive care. Documentation: Maintaining accurate and up-to-date records of resident assessments, treatment plans, and progress. Falls Prevention: Implementing strategies to reduce the risk of falls, such as balance training and environmental modifications. Pain Management: Utilizing various techniques to help residents manage pain and improve their comfort levels.
