Summary Position type: Non‑exempt, full‑time or part‑time, hourly role. Scope of role: Provide direct support within a single program or unit, typically supporting 4–6 individuals in residential settings or up to 10 individuals in periodic programs. Key responsibilities: Deliver support per individual service/program plans; assist with daily activities (personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living). Essential Job Functions Direct support: Assist with socialization, behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living. Records: Accurately document progress and activity; review records and logs; maintain confidentiality. Management of individuals’ assets: Make cost‑effective purchases within budget and according to individual preferences. Relationships: Maintain healthy professional relationships with individuals, families, guardians, and case managers; greet family members and visitors; implement Sevita customer service standards. Reporting: Report any allegations of abuse or neglect and any incidents in accordance with Sevita policy and external requirements. Rights: Maintain confidentiality, respect the rights of persons served, practice universal precautions, and assist individuals in exercising their rights. Health care: Appointments, medication administration, general health care, medical supplies and equipment, meal preparation – all to be performed accurately and with safety in mind. Employment responsibilities: Attend orientation and ongoing training, volunteer for staff meetings, adhere to scheduling, maintain teamwork, and comply with workplace safety policies. Maintenance: Transport individuals, perform housekeeping, make minor repairs, maintain safety, and report any hazards. Other: Perform other duties as required and use Mobile Punch Authorized for timekeeping. Supervisory Responsibilities None Minimum Knowledge and Skills Required for the Job Education and Experience High school diploma or equivalent preferred. Six months of experience in human services preferred. Must be 18 years of age. Working knowledge of computers. Certificates, Licenses, and Registrations Valid driver’s license in good standing. Car registration and vehicle insurance if providing transportation for individuals receiving services. All state‑required training and certification completed within mandated timeframes. Maintain valid driver’s license, meet all insurance requirements, and be approved by management to be a driver for the company. Work Environment Works at the program location and may accompany individuals into the community. Physical Requirements Heavy work: Exert up to 100 pounds of force occasionally, 50 pounds of force frequently, and 20 pounds of force constantly to move objects. Employees must cooperate with licensee and Department of Human Services staff during inspections, inquiries, or investigations. All employees must obtain drug testing, criminal background checks, and other required screenings before and during employment. Employees at least 18 years of age are required to comply with all training and proficiency demonstrations.
