About the Role:

Graceful Legacy Hospice in Fredericksburg, VA is looking for a compassionate and dedicated Hospice RN to join our care team. Responsibilities:

• Conduct comprehensive patient assessments and develop individualized plans of care for hospice patients
• Manage pain and symptom control to maximize patient comfort and quality of life
• Coordinate care with interdisciplinary team members including physicians, social workers, and chaplains
• Provide education and emotional support to patients and families regarding the hospice process
• Perform skilled nursing visits in patient homes, assisted living facilities, or care settings throughout the Fredericksburg, VA area
• Accurately document patient care, medication administration, and clinical notes in compliance with hospice regulations
• Respond to after-hours calls and provide on-call nursing support as needed

Requirements:

• Active RN license in the state of Virginia
• Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience preferred
• Strong knowledge of pain management, symptom control, and end-of-life care best practices
• Excellent communication and interpersonal skills with patients, families, and care teams
• Ability to work independently and manage a patient caseload in a field-based environment
• Valid driver's license and reliable transportation for home visits in the Fredericksburg, VA area
• Compassionate, patient-centered approach with emotional resilience in sensitive care situations
