About the Job

As a Registered Nurse Ambulatory, you'll provide professional nursing care across our Foundation practices. Working collaboratively with providers and the care team, you will deliver safe, evidence-based, patient-centered care both in-person and through telephone triage and telehealth modalities.

What You'll Do

• Carry out the nursing processassessment, planning, intervention, and evaluationacross ambulatory and telehealth settings.
• Collaborate with physicians, advanced practice providers, and interdisciplinary team members to design and implement plans of care.
• Administer medications, vaccines, treatments, and perform point-of-care testing within scope of practice.
• Educate patients on self-management, medications, nutrition, health, and wellness.
• Document care accurately in the EMR and participate in pre-visit planning for patient encounters.
• Provide leadership and mentorship to clinical staff and assist with onboarding.
• Respond to urgent or emergent situations in accordance with facility guidelines.
• Stay current on competencies, licensure, and ongoing educational requirements.

Who You Are

• Associate of Science in Nursing (ASN) required; Bachelor of Science in Nursing (BSN) preferred.
• Current Registered Nurse license in NH (or NLC state/MA) and BLS certification required.
• 1+ year of professional nursing experience preferred; ambulatory care experience a plus.
• Strong knowledge of nursing principles, EMR systems, and patient-centered care models.
• Skilled communicator with excellent time management, adaptability, and teamwork abilities.
• Compassionate, detail-oriented, and able to thrive in a fast-paced, collaborative environment.

Monday, Tuesday, Wednesday and Friday 7:30 am start
