This field‑based role offers a 4‑day or 5‑day schedule and a range of benefits. 5k Sign‑On Bonus Competitive salary & annual bonus Field schedule: 4 days 10 hours or 5 days 8 hours No weekends, holidays, or night shifts 24 days PTO, 29 days after 1 year of employment Comprehensive benefit plan options Extensive 4‑6 week training program Community‑based nursing Dedicated, supportive & tenured team Job Summary As an Assessment RN, you will visit members in their homes at intervals required by the Department of Health (DOH) and complete appropriate documentation to determine members’ enrollment, service needs, and continued plan eligibility. The position coordinates with the Care Management Team to maintain efficient and effective operations of Healthfirst’s long‑term care plans such as CompleteCare, FIDA, and Senior Health Partners. Responsibilities Conduct assessments for prospective members and re‑assessments of current members, identifying enrollment or eligibility using clinical expertise and critical thinking skills. Conduct assessments in settings other than home, such as skilled nursing facilities, as needed. Conduct in‑person care management visits to support person‑centered service planning. Complete assessments of members’ needs due to changing conditions. Review previous nursing assessments/medical notes to determine necessary adjustments or updates in Care Plans and/or to assist with nursing home placement eligibility. Coordinate with home care agencies, vendors, pharmacies, medical providers, and the Care Management Team regarding identified needs. Provide community‑based education for frail populations, including self‑care and preventative care. Perform additional duties as assigned, including care management tasks as needed. Minimum Qualifications New York State RN license. Ability to travel in and around the Bronx, New York. Unexpired NY state driver’s license or NYS ID. Preferred Qualifications Geriatrics Home Care Discharge Planning Case Management Medical Surgical Nursing Electronic PHI database experience Knowledge of health insurance, Medicaid, Medicare and MLTCP Experience with frail adult or elderly populations, including determining appropriate care plans and negotiating initial service plans. Time management, critical/creative thinking, communication, and problem‑solving skills. Field experience assessing, planning, and evaluating member or patient care. Demonstrated ability to handle heavy caseloads. Intermediate Microsoft Word, Excel, and Outlook skills. Salary & Benefits Hiring Range: Greater New York City Area (NY, NJ, CT residents): $81,100 – $117,470 All other locations: $71,600 – $106,505 Benefits include medical, dental, and vision coverage, incentive and recognition programs, life insurance, and 401(k) contributions (subject to eligibility requirements).
