Case Manager Registered Nurse

What We Offer Position: RN Case Manager Schedule: Monday–Friday, 8:00 AM–5:00 PM Location: Forsyth Medical Center, Winston-Salem, NC Dept: Neuro Unit Join our Neuro Unit as an RN Case Manager and make a meaningful impact beyond the bedside. In this full-time, day-shift role, you’ll help patients and families navigate complex healthcare needs while leading safe, coordinated transitions of care. Monday–Friday, 8:00 AM–5:00 PM schedule Leadership role in complex discharge planning and care coordination Collaborative Neuro care team working alongside physicians, nurses, MSWs, and other healthcare professionals Meaningful patient impact through individualized, proactive, and safe discharge planning Support for medically complex neurological patients and their unique care needs Collaboration with MSWs on psychosocially complex cases Professional growth through exposure to complex neurological conditions, post-acute care, and community resources Advocacy and coordination to help patients and families successfully transition to the next level of care Supportive, mission-driven environment focused on quality, safety, and patient-centered care If you’re an RN who enjoys advocacy, problem-solving, collaboration, and helping patients navigate complex healthcare journeys, come make a difference on our Neuro team. What We're Looking For Education: 2 Year / Associate Degree, required. Associates Degree in Nursing or BSN. 4 Year / Bachelors Degree, preferred. Experience: Two or more years healthcare experience, preferred. Case Management or related experience, preferred. Licensure/Certification: Current RN licensure in state of employment, required. Additional Skills (required): Weekend and holiday coverage, if applicable. Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding. Why Choose Novant Health?
