VA or VA Partner Experience is Required:

• This position requires prior experience working within the Department of Veterans Affairs (VA) system or current experience with a VA partner organization such as Optum Serve or a similar organization , along with demonstrated program management experience.
• Former VA employees are strongly encouraged to apply, particularly professionals with backgrounds in VA care coordination, social work, Veteran benefits, or Vet Center programs.

Important: Due to a contractual restriction, candidates who are currently employed directly by the Department of Veterans Affairs are not eligible for consideration.

Residency in One of Four States is Required:

• This is a fully remote position; however, candidates must reside in Alabama, Mississippi, Louisiana, or Tennessee .
• The position follows a Central Time schedule of 8:30 AM – 4:30 PM CST, Monday through Friday.

Program Management Experience is Required:

• This is more than a traditional care coordination position. The successful candidate will help develop and establish workflows, processes, and best practices for a Veteran-focused community care program.
• Candidates must have experience within the VA system or with a VA partner organization and demonstrated progressive program management experience.
• Care coordination experience alone does not meet the requirements for this position.

About the Role:

We are seeking an experienced Veteran Care Coordination Specialist with a strong background in VA healthcare, social work, behavioral health, and program development.

This individual will serve as a subject matter expert in helping Veterans navigate complex healthcare systems, VA and Community Care programs, benefits, behavioral health resources, and essential community services.

The role combines Veteran advocacy, care coordination, social work expertise, program development, workflow design, and team education.

You will play a key role in establishing effective processes for a Veteran-focused community care program while providing guidance on complex cases and helping care coordination teams effectively navigate the unique needs of the Veteran population.

Reports to: Director of Care Coordination.
Supervisory Responsibility: None.

What You'll Do:

Program Development & Workflow Design:

• Help develop and refine Veteran-specific care coordination workflows, processes, and best practices.
• Establish guidelines for complex cases, referrals, escalations, and resource navigation.
• Help define appropriate responsibilities and scopes of practice for Care Coordinators, nurses, and social workers.
• Identify operational gaps and opportunities to improve Veteran services and the overall care experience.
• Help build processes that improve access to healthcare, community resources, and supportive services.
• Contribute to the ongoing development and evolution of the Veteran-focused program.

Veteran & VA Subject Matter Expertise:

• Serve as a subject matter expert regarding VA healthcare, Community Care, Veteran benefits, and available resources.
• Guide Care Coordinators through VA systems, programs, benefits, and community-based services.
• Educate Veterans and team members about available healthcare and social support resources.
• Help connect Veterans with appropriate healthcare providers, community organizations, and support services.
• Maintain current knowledge of Veteran healthcare programs, benefits, behavioral health resources, and social services.

Complex Case & Care Coordination Support:

• Provide guidance and expertise on complex Veteran cases.
• Assist in determining appropriate referrals, resources, and escalation pathways.
• Help address barriers related to healthcare access, benefits, behavioral health, housing, food insecurity, and other social needs.
• Provide direct support to Veterans when specialized expertise or intervention is required.
• Document case activity, interactions, and recommendations accurately within applicable care management or electronic health record systems.

Training & Team Development:

• Partner with leadership to develop and enhance Veteran-specific training programs and educational materials.
• Educate Care Coordinators on VA healthcare, Community Care, benefits navigation, Veteran resources, and common challenges affecting Veterans.
• Serve as an ongoing Veteran, VA, and social work resource for the care coordination team.
• Help strengthen team knowledge, confidence, and effectiveness when supporting Veteran populations.
• Provide guidance as new workflows, programs, and processes are implemented.

Required Qualifications:

• 5+ years of relevant professional experience as a Clinical Social Worker, Care Coordinator, Care Navigator, or similar healthcare/human services professional.
• Demonstrated progressive program management experience .
• Previous experience working directly with Veterans and/or within the VA healthcare system.
• Previous employment with the Department of Veterans Affairs or current employment with a VA partner organization such as Optum Serve or a similar organization .

• Strong knowledge of:

  • VA healthcare.
  • Community Care.
  • Veteran benefits and resources.
  • Behavioral health.
  • Social determinants of health.
• Experience supporting medically complex and/or behavioral health populations.
• At least 1 year of successful remote work experience .
• Experience communicating with members or patients by telephone and text message.
• Experience documenting case notes within a care management platform, EHR, or similar system.
• Experience utilizing motivational interviewing techniques.
• Excellent verbal and written communication skills.
• Strong critical thinking, problem-solving, collaboration, and relationship-building skills.
• Ability to work independently while maintaining effective communication with a remote team.
• Dedicated, professional home office suitable for remote work.

Preferred Qualifications:

• Experience working with Apple computers, including MacBook systems .
• Bilingual English/Spanish fluency.

This position requires a dedicated home office and the ability to work independently in a remote environment. The role involves extended periods of sitting, computer use, telephone communication, and electronic documentation.
