Hospice RN Case Manager

Join a compassionate team as a Hospice - Registered Nurse Case Manager, dedicated to providing End of Life Care to patients and their families. Key Responsibilities Assess patient preferences, values, and cultural needs to develop personalized care plans. Implement, document, and communicate individualized plans of care in collaboration with the interprofessional team, patients, and families. Promote respectful, equitable, and empathetic interactions with diverse populations, ensuring patient-centered care. Monitor changes in patient conditions, update care plans, and collaborate with the team to ensure quality outcomes. Provide education and support to patients and caregivers, advocating for their needs and preferences. Qualifications Registered Nurse (RN) with a Nursing Diploma/Associate's degree; Bachelor's preferred. Minimum of 2 years of nursing experience; experience in home health or hospice is a plus. Valid Driver's License and ability to provide coverage across regions during on-call. Registration with the Family Care Safety Registry required. Skills & Attributes Strong assessment, critical thinking, and communication skills. Ability to work collaboratively within a multidisciplinary team. Commitment to culturally competent and compassionate care.
