Registered Nurse Hospice LiaisonLocation: Essex CountySchedule: Full TimePosition Summary: The VNA Health Group is seeking a compassionate and experienced Registered Nurse Hospice Liaison to join our dedicated Hospice Team. This vital role focuses on assessing patients for hospice eligibility, coordinating their transition to hospice care, and conducting patient admissions. The Registered Nurse Hospice Liaison will be responsible for covering the following hospitals: Newark Beth Israel Medical Center, Clara Maass Medical Center, Cooperman Barnabas Medical Center, and Jersey City Medical Center.
