Seven Healthcare are seeking a skilled Physical Therapist for a contract position in Morgantown, WV. This role involves providing physical therapy evaluation and treatment to patients based on physician referrals. - Complete comprehensive evaluations using appropriate assessments and tools.
- Review patient records, including medical history and precautions.
- Establish measurable goals and develop treatment plans for patient progress.
- Regularly reassess treatment effectiveness and patient progress towards goals.
- Communicate patient needs and progress to the treatment team and relevant parties.
- Consult with medical providers regarding patient evaluations and treatments.
- Recommend and assist in securing adaptive equipment for patients.
- Maintain and submit documentation as required by regulations.
- Supervise Physical Therapist Assistants and Aides.
- Participate in staff development programs and maintain clinical practice skills.
- WV PT license
- Home Health experience with OASIS preferred
- Experience with HCHB preferred
