Required Qualifications

Experience

• Minimum 2 years of Acute Medical Care Management experience
• Experience performing:
  • Care Coordination
  • Patient Assessment
  • Discharge Planning
  • Treatment Planning
• Experience must be recent, within the last 3 months
• Prior travel nursing experience required ( Hard Stop )
• Acute care hospital experience required

Charting

• Epic experience required (Hard Stop)

Licensure

• Active California RN License required
