About the Role:
The Senior Patient Navigator provides clinical leadership, patient navigation, and care coordination to ensure patients receive timely, high-quality, and equitable care. This role serves as a liaison between patients, healthcare providers, and community resources while supporting clinical operations, staff training, quality improvement, and program compliance.
Minimum Qualifications:
- Active and unrestricted Registered Nurse (RN) license.
- BSN preferred.
- Minimum of 5 years of nursing experience.
- Experience in patient navigation, care coordination, case management, or clinical program management.
- Strong communication, leadership, organizational, and problem-solving skills.
- Experience with Electronic Medical Records (EMRs).
Preferred Qualifications:
- Bilingual (English/Spanish).
- Experience working with diverse and underserved populations.
- Experience in staff training, quality improvement, or regulatory compliance.
Responsibilities:
- Coordinate patient care, referrals, appointments, and follow-up services.
- Educate and support patients and families throughout their healthcare journey.
- Collaborate with physicians and interdisciplinary teams to improve care coordination and patient outcomes.
- Provide clinical guidance and evidence-based recommendations to staff and leadership.
- Train and mentor staff on patient navigation, clinical best practices, and program requirements.
- Maintain accurate patient documentation and ensure compliance with clinical and regulatory standards.
- Monitor program performance, quality measures, and patient outcomes while supporting continuous improvement initiatives.
- Participate in meetings with leadership, funders, and community partners to advance program goals.
- Advocate for patients by addressing barriers to care and connecting them with appropriate community resources.

The Senior Patient Navigator utilizes advanced clinical nursing knowledge, patient navigation, and care coordination skills to improve patient outcomes and ensure seamless access to healthcare services. This role requires strong communication, interpersonal, and patient education abilities to effectively support patients, families, and multidisciplinary care teams. The Senior Patient Navigator demonstrates leadership through staff training, clinical consultation, and mentoring while applying critical thinking and sound clinical judgment to resolve complex patient care issues. Proficiency with Electronic Medical Records (EMRs), healthcare technology, and data analysis supports accurate documentation, quality improvement initiatives, and evidence-based practice. Success in this role also requires strong organizational and time management skills, knowledge of healthcare regulations and compliance standards, and the ability to collaborate effectively with providers, leadership, community partners, and funders while delivering culturally responsive, patient-centered care.
