Schedule / Job Type

Contract | Full-Time | Days | Monday–Friday, 8:00 AM–5:00 PM | Onsite

Key Responsibilities

• Provide skilled physical therapy services to patients within a home health setting across an assigned territory
• Conduct evaluations, develop individualized treatment plans, and deliver therapeutic interventions in the patient's home
• Maintain a productivity standard of 30 points per week (approximately 6 points per day)
• Document patient care accurately and in a timely manner using Homecare Homebase (HCHB)
• Collaborate with interdisciplinary care team members to support patient goals and outcomes

Required Qualifications

• Active Physical Therapist (PT) license in the state of Oregon
• Previous home health experience required
• Previous Homecare Homebase (HCHB) experience required
• Ability to travel within a 50-mile radius of the office location

Preferred Experience / Skills

• Experience managing a caseload independently in a home health environment
• Strong time management and organizational skills to meet daily productivity expectations
