The Opportunity:
The Multnomah County Health Department’s Primary Patient Care team is looking for a Licensed Practical Nurse who deeply desires to make a difference in their community.

This position provides focused community and clinic health services towards the prevention of disease, the promotion of health, the reduction of disability, improving the client’s self-care, using professional judgment in focused/limited assessments, screening, contributing to care planning, intervention and provides health information/education to patients. We’re seeking a visionary and dynamic leader who is creative in problem solving and contributes new ideas and solutions to the organization to join our team in Integrated Clinical Services
The Licensed Practical Nurse works within the health care team and provides focused community and clinic health services directed towards the prevention of disease, the promotion of health, the reduction of disability, improving the clients self-care, using professional judgment in focused/limited assessments, screening, contributing to care planning, intervention, and provides health information/education to patients. The Panel Manager role tracks the health of the population of patient's assigned to the provider panel. Manages team communication including phone messages, fax requests, and warm hand-offs, as needed. The Panel Manager is responsible for the team InBasket within the electronic health record system and delegates requests to appropriate staff, in addition to completing those requests that are within the scope of a LPN. The Panel Manager completes needed patient visits as noted on EPIC team schedule in addition to providing focused assessments to walk-in clients. Supports providers when staffing indicates the need. Panel Management
Tech-Savvy Efficiency : Leverages electronic reports and scheduling systems to proactively identify and address patient needs for preventive screenings, ensuring timely care and improved health outcomes.
Patient-Centric Preparation: Proactively conducts pre-visit activities such as chart reviews and team huddles, ensuring each patient encounter is well-prepared and personalized for optimal care.
Collaborative Chronic Disease Management: Partners with providers and the care team to actively monitor chronic disease and prevention reports, initiating timely follow-ups or referrals to optimize patient health and well-being.
Team/Client Communication
Communication Powerhouse: Seamlessly manages multiple communication channels (phone, email, in-person), ensuring clear and effective information flow across the care team and with patients, in any language utilizing interpreters if needed..
Organization & Efficiency Expert: Masters both electronic and paper systems, expertly handling inboxes, scheduling, and follow-ups to keep the team running smoothly and patients well-informed.
Patient-Centric Advocate: Proactively participates in reminder calls, follows up on no-shows, and ensures patients receive critical information about referrals, treatments, and medications.
Detail-Oriented Documenter: Maintains accurate and comprehensive patient records, utilizing SOAP notes, EPIC phrases, and clear communication to support high-quality care and collaboration.

Dynamic Clinical Support: Provides comprehensive clinical assistance to providers, performing a wide range of tasks from patient intake and vitals to administering medications and assisting with procedures.
Patient-Focused Care: Delivers compassionate and personalized care, performing focused assessments, providing patient education, and offering post-visit support to ensure optimal outcomes.
Skilled Multitasker: Juggles diverse responsibilities seamlessly, from performing diagnostic tests and administering treatments to completing documentation and assisting with referrals.
Team Player: Contributes to a collaborative care environment, communicating effectively with patients, providers, and colleagues to ensure exceptional patient experiences.

Minimum Qualifications/Transferable Skills*:
An Associate's degree or a Bachelor's degree in nursing from an accredited nursing program
Possession of, or ability to obtain, an appropriate and valid license to practice as a Licensed Practical Nurse issued by the State of Oregon by the time of hire
A minimum one (1) year of nursing experience
BLS/CPR certification required
