About the Position

Job Title: Case Manager RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day 5x8-Hour Hours per Shift: 8 hours Experience: Minimum 1 year License: AK RN license required Certifications: BLS preferred, ACM certification preferred Must-Have: Must have Epic experience. Must have acute care experience. Description: Case Manager RN position available. Candidates must have an AK RN license in hand for auto offer. The position requires working during the day shift, typically from 0700 to 1700. Candidates who are first-time travelers are accepted. Weekend work is required, with every other weekend or 4 weekend days in a 4-week schedule. The position do not require being on-call. Candidates will float within their scope to meet facility needs. The facility is a Magnet facility with a Level II Trauma designation. Daily census averages 324 patients. The position involves discharge planning and care coordination. Candidates must have knowledge of CMS and the ability to determine medical necessity per evidence-based guidelines. Experience in using Epic as a charting system is required. Candidates should provide care across diverse patient age groups, including neonates, adolescents, and geriatrics. Candidates must be open to meeting the department's scheduling needs, which include requiring weekend coverage. References covering recent experience are required.
