Responsibilities
- Perform comprehensive patient evaluations and establish individualized physical therapy plans aimed at restoring function and improving mobility.
- Deliver therapy interventions using evidence-based techniques and established protocols; monitor progress and adjust care as needed.
- Collaborate as a co-leader within an interdisciplinary team to support patient groups and discharge planning.
- Document evaluations, treatment plans, progress notes, and discharge summaries with timely, accurate clinical documentation.
- Supervise and delegate appropriate tasks to assigned therapy assistants; ensure compliance with safety and quality standards.
- Communicate care plans and progress to patients, families, and relevant care team members to support coordinated care.

Requirements
- Education: Doctor of Physical Therapy (DPT) degree; required if license obtained on or after January 2016 or foreign equivalent.
- Licenses/Certifications: Physical Therapy licensure in all applicable states of practice; CPR certification or BLS (or equivalent) from an approved vendor per policy.
- Experience: At least two years of recent experience in physical therapy.
- Certifications/Other: Current BLS certification; must hold a valid driver’s license and be at least 21 years of age.
- Other requirements: Current nursing license per the state of assignment; EPIC EMR experience preferred; Day shift availability.
