As a Home Health Licensed Master Social Worker, you'll help patients and families navigate life's challenges by identifying barriers to care, connecting them with community resources, providing emotional support, and helping them make informed decisions about their health and well-being.

Your work can ease burdens, strengthen families, and improve quality of life when it matters most.

What You'll Do

• Complete psychosocial assessments for home health patients
• Identify social, emotional, environmental, and financial needs
• Connect patients and families with community resources and support services
• Assist with long-term care planning and discharge planning
• Provide counseling, education, and emotional support to patients and caregivers
• Collaborate with physicians and interdisciplinary team members
• Maintain timely and accurate documentation

Qualifications

• Active Virginia Licensed Master Social Worker (LMSW) License Required
• Master's Degree in Social Work from an Accredited Program
• Valid Driver's License and Reliable Transportation
• Strong Communication, Assessment, and Documentation Skills
• Home Health or Healthcare Experience Preferred but Not Required
• Passion for Helping Patients and Families Navigate Complex Challenges
