Interventional Radiology Nurse Navigator

The Interventional Radiology Nurse Navigator is a registered nurse dedicated to coordinating the complex care needs of patients undergoing Interventional Radiology procedures. This role requires strong organizational and communication skills to manage service access, diagnostic and treatment scheduling, and to facilitate multidisciplinary collaboration. The nurse navigator serves as the primary contact for patients and families, ensuring seamless transitions of care, providing education, and supporting the entire care process with a compassionate and patient-centered approach.

Interventional Radiology experience required. Day shift.

Essential Functions

• In collaboration with the interdisciplinary team, help create a tailored health care plan for assigned patients.
• Develop an individualized plan with the MD for appropriate services for the patient.
• Reports pertinent observations and reactions regarding clients to the appropriate person.
• Assesses and coordinates patient's discharge planning needs with members of the healthcare team.
• Resolves patient problems and needs by utilizing multidisciplinary team strategies.
• Educates patients regarding how to navigate throughout the health system.

Qualifications

Education: Bachelor's Degree Nursing required or Master's Degree Nursing preferred.

Licenses and Credentials: Registered Nurse [RN - State License] - Generic - HR Only required.

Experience: Experience in an acute care setting and/or provider practice and/or experience in a community-based role or with discharge/transitional planning background 2-3 years preferred.

Knowledge, Skills and Abilities: Strong attention to detail and organization. Strong cultural awareness competencies. Proficiency in Microsoft Office. Maintains good communication skills. Readily adapts to change and welcomes new approaches when circumstances demand it.
