We strive to attract professionals who believe in our mission, vision and are dedicated to the service of our patients and their families creating a memorable experience through compassion, respect, and kindness.”Job SummaryBehavioral Health Counselors are core members of the Sanitas medical team and provide hope, empowerment, and psychoeducation in order to promote autonomy and self-management by arranging comprehensive mental health care services in a primary care setting.Essential Job FunctionsReasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Develop and implement care plansComplete and maintain all documentation in a timely and accurate manner according to all federal, state and center guidelinesCollaborate with a variety of health professionals in the patient’s circle of care including but not limited to primary care providers, care nurses, care coordinators, community health workers, psychiatrists, and other mental health professionals and specialistsEnsure a comprehensive continuity of careProvide individual, family or group educational interventionsGuide patients in the development of skills and strategies for dealing with their diagnosisAssist patients in adjusting to life and making changesParticipate in regularly scheduled weekly caseload consultation and complex case review meetingsFacilitate care plan adjusts for patients who are not improving as expected in consultation with the medical providersFacilitate referrals to the appropriate provider or in-house intervention program as clinically indicatedConnect patients and family members with community and social resourcesGuide clinical staff on how to manage common behavioral health situationsScreen and assess patients for common mental health and substance abuse disordersFacilitate patient engagement and follow-up careSupport psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatmentPromote early detection, accurate diagnosis, personalized treatment, and ongoing support, ultimately leading to improved overall well-being for individuals facing these challengesTrack treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complicationsProvide or facilitate in-clinic or outside referrals to evidence-based psychosocial as clinically indicatedGuide clinical staff on how to manage common behavioral health situationsResolve emergency problems in crisis situationsThe position includes providing behavioral health services via telehealth, consistent with state and federal guidelinesSupervisory ResponsibilitiesThis position has no supervisory responsibilities.Required EducationMaster’s degree in counseling or related field (Social work, Mental health counseling, marriage and couples’ therapy) from accredited program.Required ExperienceRelevant experience 1 year.Required Licenses and CertificationsBLS certification.Required Knowledge, Skills, and AbilitiesAble to establish and maintain effective working relationships with mentally or emotionally disabled persons and their families.Flexible schedule weekdays – weekends.Good communication and listening skills.Ability to show empathy and compassion.Preferred QualificationsPreferably 1 year of experience providing CBT-based interventions with adults with depression and anxiety.Registered Mental Health Counselor Interns are preferred.Financial ResponsibilitiesThis position does not currently have financial responsibilities.Budget ResponsibilitiesThis position does not currently have budget responsibilities.LanguagesEnglish is required.Spanish is preferred.TravelThis position requires the ability to travel in the assigned region.Physical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.Includes all patient-facing clinical staff ( Medical Assistants, Phlebotomists, Mammography Technologists, Radiologic Technologists, Multi-Specialty staff, Providers) as well as Facilities Technicians. These positions typically require prolonged standing and walking, frequent bending, reaching, lifting, pushing, and pulling of equipment or supplies (up to 50 pounds), manual dexterity, and compliance with infection control and safety protocols. Facilities Technicians may also perform repairs, renovations, remodeling, preventive maintenance, and use tools and equipment to support our medical centers.Environmental ConditionsInside: The employee is subject to environmental conditions, protection from weather conditions but not necessarily from temperature changes.
