How you'll make an impact in this role

• Coordinate the overall interdisciplinary plan of care from admission to discharge, integrating patient and family preferences, while requiring an LSW or LCSW to manage the process.
• Monitor patient care through assessments, evaluations, and medical records while acting as a resource for documentation accuracy and treatment appropriateness.
• Advocate for necessary resources, remove barriers, and coordinate options for post-discharge care.
• Maintain ongoing dialogue with supervisors and other healthcare providers to ensure effective implementation of the health plan.

What minimum requirements you'll need

Licensure / Certification / Registration:

• One or more of the following required:
  • Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate'
  degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
