- Coordinate the overall interdisciplinary plan of care from admission to discharge, integrating patient and family preferences, while requiring an LSW or LCSW to manage the process.
- Monitor patient care through assessments, evaluations, and medical records while acting as a resource for documentation accuracy and treatment appropriateness.
- Advocate for necessary resources, remove barriers, and coordinate options for post-discharge care.
- Maintain ongoing dialogue with supervisors and other healthcare providers to ensure effective implementation of the health plan.
- One or more of the following required:
- Clinical Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
- Counselor credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
- Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
- Therapist specializing in Marriage & Family credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
- High School diploma equivalency with 2 years of cumulative experience OR Associate' degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
