Work Environment\n

This position offers the flexibility of remote work while serving clients affiliated with a hospice center in Jersey City, New Jersey. The work environment is collaborative and supportive, allowing you to engage with clients through virtual platforms while also accommodating occasional in-person visitations when necessary. A typical full-time schedule runs from Monday to Friday, 8:00 AM to 4:30 PM, with opportunities for evening or weekend hours as needed to meet client demands.

  • Conduct comprehensive assessments of seniors’ psychosocial needs, with a focus on emotional, mental, and social aspects pertinent to their well-being.

  • Collaborate closely with clients and their families to develop and monitor personalized care plans tailored to specific goals, addressing their unique preferences and requirements.

  • Provide emotional support and counseling, assisting clients in navigating life transitions, managing grief, and overcoming challenges associated with end-of-life concerns.

  • Advocate for clients’ rights and connect them to essential community resources, ensuring uninterrupted access to vital services and support.

  • Work in tandem with an interdisciplinary team, including nurses, caregivers, and medical professionals, to assure cohesive and holistic care for clients.

  • Perform regular check-ins to monitor clients’ conditions, evaluate their living environments, and identify any safety concerns that may arise.

  • Facilitate effective communication within families by resolving conflicts and ensuring all parties feel heard and supported in decision-making processes.

  • Educate clients and families about community resources, support groups, and programs that can enhance social engagement and overall quality of life.

  • Maintain meticulous records of client assessments, care plans, interaction notes, and other relevant documentation to ensure compliance with regulatory standards.

  • A Master's degree in Social Work (MSW) from an accredited institution.
