Work Environment

This full-time role is based within an outpatient office, focusing on delivering care in various home environments. You will work Monday to Friday from 9 AM to 5 PM, providing you with a structured schedule while also allowing you to experience the rewarding nature of working directly with patients in their own homes. Key Responsibilities

• Perform comprehensive in-home assessments of patients to evaluate their physical conditions and mobility challenges.
• Design and implement individualized treatment plans incorporating therapeutic exercises, gait training, and balance retraining to enhance recovery outcomes.
• Educate patients and their caregivers on prescribed treatment plans, including home exercises and strategies for managing symptoms or improving overall mobility.
• Work collaboratively with a multidisciplinary team, including physicians and nurses, to ensure optimal patient care and successful rehabilitation results.
• Maintain accurate and timely documentation of patient progress and treatment interventions in compliance with Medicare guidelines and agency policies.
• Travel within a designated territory to deliver care directly to patients, adapting treatment settings according to their unique home environments.

Qualifications

• A current and valid Physical Therapist license in the state of New Jersey is required.

Compensation and Benefits
