AM shift from 8 a.m. to 4 p.m., may require weekend coverage. Job Description The In-Patient Case Manager coordinates patients through the acute care hospitalization, managing all aspects of the discharge planning and transition process. The role involves performing utilization management and quality screenings. Responsibilities Coordinate patients through the acute care hospitalization process. Manage all aspects of discharge planning and transition processes. Perform utilization management and quality screenings. Required Experience / Certifications / Licensure 2 years of acute care experience strongly preferred. Experience in Case Management and Managed Care required. BSN required.
