Family Physician Assistant House Calls / In-Home Primary Care Location: Brooklyn, New York Position Type: Full-Time, Permanent (Direct Hire) About the Opportunity An established home-based primary care medical group in Brooklyn, NY, is seeking a qualified House Call Physician Assistant to join their growing clinical team. This full-time, direct-hire position offers an autonomous practice environment with manageable daily patient volume, dedicated administrative support, and flexible scheduling, allowing you to provide comprehensive, patient-centered care to adult and geriatric populations in their homes. Key Responsibilities Conduct comprehensive in-home medical assessments, routine physicals, and ongoing primary care management for adult and geriatric patients. Diagnose and manage acute illnesses and complex chronic conditions (e.g., hypertension, diabetes, CHF, COPD, neurodegenerative disorders) in the home setting. Order, interpret, and review laboratory work, diagnostic testing, and mobile imaging studies (X-rays, ultrasounds). Formulate evidence-based treatment plans, prescribe medications, and manage home health/durable medical equipment (DME) orders. Collaborate closely with supervising physicians, home health agencies, physical therapists, and family caregivers to ensure seamless care coordination. Maintain prompt and accurate clinical documentation using mobile electronic health record (EHR) systems. Qualifications Graduation from an accredited Physician Assistant (PA) Program. Active, unencumbered New York State Physician Assistant License. NCCPA Certification (PA-C). Active New York DEA registration and Prescriptive Authority.
